COD Annual Reporting Tool

The Experimental Sites Initiative (ESI) COD Annual Reporting Tool will be used to meet reporting requirements for participants as outlined in the PPA Amendment for the experiments listed below.

• Second Chance Pell
• Federal Work-Study (FWS)

Participating institutions will also be sent a link to an annual survey related to the experiment. Survey completion is also part of the reporting requirements. The link to institutions will be sent from the ESI mailbox. FSA has chosen to utilize Survey Monkey to administer the survey. The survey link will be sent at the conclusion of the award year.
